Tragic Violence Erupts in Merrylands: Man Charged Following Fatal Stabbing

A shocking incident of violence has shaken the Sydney suburb of Merrylands, resulting in the death of one person and critical injuries to two others. Setefano Mooniai Leaaetoa, a 25-year-old man, has been charged with murder and wounding with intent to murder following an alleged stabbing spree on Tuesday morning. The alleged attacks, which reportedly involved a kitchen knife, occurred on Merrylands Road, leaving a 38-year-old man deceased at the scene. A 47-year-old woman and a 21-year-old man are currently in a critical condition in hospital, battling for their lives.

The arrest of Mr Leaaetoa comes under a dark cloud, with revelations that he had recently absconded from a mental health facility. The Western Sydney Local Health District (WSLHD) confirmed that Mr Leaaetoa had escaped their care a mere 10 days prior to the alleged fatal incident. He was being transferred between hospitals when he made his escape.

Questions Arise Over Mental Health Patient’s Escape

A spokesperson for the WSLHD stated that the individual, alleged to be involved in the Merrylands attack, was a recent patient at Cumberland Hospital, a facility that provides acute inpatient care for individuals with complex mental health needs. The escape occurred on February 7th, when the patient absconded while being transferred from Cumberland Hospital to the Westmead Hospital emergency department for further medical assessment.

Following the escape, the NSW Police were notified, and efforts were made to contact the patient’s family. Despite these attempts, the WSLHD was unsuccessful in re-establishing contact with the individual. In response to the incident, the WSLHD has announced a formal review into the patient’s care and treatment, which will include the involvement of an external senior psychiatrist.

The Mental Health Minister, Rose Jackson, has also indicated that a thorough review will be conducted into the circumstances surrounding the man’s escape from the care of health professionals. Minister Jackson noted that initial reports suggest a security guard was present during the transfer, and that standard protocols were followed. However, she emphasised that a full review is underway to identify any potential lessons learned.

Court Proceedings and Previous Charges

On Wednesday, Mr Leaaetoa appeared before Parramatta Local Court. He did not apply for bail, indicating he will remain in custody. The court also heard that Mr Leaaetoa is facing additional charges related to a separate domestic violence incident, including assault occasioning actual bodily harm.

Apprehension of Suspect

Police were alerted to the unfolding tragedy around 10 am on Tuesday, responding to reports of multiple stabbings on Merrylands Road. Witnesses reportedly informed officers that a man had allegedly attacked several people inside a business before fleeing the scene.

The accused was apprehended by police a short time later on Smythe Street, a neighbouring street to the alleged crime scene.

Investigation into the Attack

Superintendent Simon Glasser, speaking to reporters on Tuesday, provided some initial details about the suspect. He stated that Mr Leaaetoa was primarily known to police for petty crime and had multiple mental health reports on record. Investigators believe a kitchen knife, measuring between 20cm and 30cm in length, was used in the attack within the suburban shopping precinct.

Mr Leaaetoa was transported to Granville police station. On Wednesday morning, he was formally charged with one count of murder and two counts of wounding with intent to murder. The investigation into the motive and the precise sequence of events leading up to and during the alleged stabbings is ongoing. Authorities are appealing to the public for any further information that may assist their inquiries.
